A Shadow Over the Presidency: Questions Surround Biden’s Late Cancer Diagnosis

Former aides to President Joe Biden expressed shock and skepticism to the New York Post following the announcement of his stage 4 prostate cancer diagnosis, revealed just two days ago on Sunday.

The news, which came after Biden reported “increasing urinary symptoms,” has sparked intense speculation among those who worked closely with the 82-year-old former president. Many are questioning why the cancer, now spread to his bones, wasn’t detected earlier—and whether concerns about public perception may have delayed critical testing.

“I do believe that they felt like something was going on, and were worried about it and kind of hedged around it for a little bit,” said a former White House staffer who worked in close proximity to Biden.

Unlike the apparent efforts to downplay Biden’s declining mental sharpness during his presidency, aides suspect the handling of his physical health stemmed from a fear of how a diagnosis might impact last year’s election. Biden, who faced mounting pressure to step aside but repeatedly refused to allow an open contest for his successor, ultimately withdrew his re-election bid on July 21 after a poor debate performance against Donald Trump on June 27.

The timing of the diagnosis has raised eyebrows, particularly as it came just before the release of Original Sin, a highly anticipated book accusing Biden’s team of concealing his mental and physical decline.

“You can’t dunk on a man with cancer,” one ex-aide noted, suggesting the announcement may have been strategically timed to soften the narrative. “[Original Sin co-authors Jake] Tapper and [Alex] Thompson have to be more careful now. They were gearing up for a steamroll.” Another longtime aide dismissed this theory by explaining that Biden’s post-presidential team is “not that cynical and smart.”

Medical experts have weighed in, emphasizing the likelihood that Biden’s cancer was present for years. “He did not develop it in the last 100, 200 days. He had it while he was president,” said oncologist Dr. Zeke Emanuel, a former Biden COVID-19 advisory board appointee, during an appearance on MSNBC’s Morning Joe. “He probably had it at the start of his presidency in 2021. I don’t think there’s any disagreement on that.”

Questions have also arisen about why Biden’s presidential physician, Dr. Kevin O’Connor, did not conduct a Prostate-Specific Antigen (PSA) blood test, a standard screening method for prostate cancer. The test was notably absent from Biden’s February 2024 physical report, his final one while president.

“I think the likely scenario, knowing Kevin O’Connor pretty well, is that they had a discussion on what a PSA test is, and they decided not to do one because it would likely be elevated and cause a story, since a lot of older men have high PSAs that are false positives,” a longtime aide speculated. “A blood test has a paper trail. They probably just didn’t do the test. I think it would leak if he had a PSA test they tried to bury.”

The aide added a grim reflection: “What’s ironic is that if they didn’t give the president a PSA test because of the potential for optics of a possible false positive, that decision is ultimately going to lead to his death because they could have caught it earlier.”

Doubts persist about whether O’Connor, affectionately known as “Doc,” was aware of any symptoms during Biden’s presidency. “Frankly, residence staff talk. It would have gotten out,” stated the former White House staffer.

I just know Doc could not have lived with himself if he knew this was happening and he didn’t do everything he could. Also, Doc isn’t discreet. Yes, he’s a doctor, but he’s always chatting.”

The close relationship between Biden and O’Connor, who also served as his physician during his vice presidency, has come under scrutiny. “They are two sides of the same coin,” one source said. “Between the two of them, someone would have slipped.”

Yet, some aides remain skeptical that Biden hid symptoms, noting his openness about physical needs. “Biden would be like, ‘Where’s the john?’ He’s a normal person,” a former campaign aide recalled, contrasting Biden’s demeanor with Vice President Kamala Harris, who preferred the term “secondary hold” over “bathroom.”

Despite Biden’s disclosure of urinary symptoms—a common indicator of prostate issues—former aides said they were unaware of frequent bathroom use during his time in office. Only a small circle, including the director of Oval Office operations and deputy chief of staff Annie Tomasini, would likely have noticed such symptoms in the West Wing.

The diagnosis has added a layer of tragedy to Biden’s already storied life, marked by personal and political challenges. “It is so sad,” one former aide said. “Joe Biden has had so much tragedy in his life.” As questions linger about the handling of his health, the former president’s legacy now faces a new, somber chapter.
